Job Title: Home Health Aide

**Job Summary:**

We are seeking a compassionate and skilled Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will provide high-quality patient care and support to individuals in their homes, helping them to maintain their independence and quality of life.

Key Responsibilities:

  • Provide direct patient care under the supervision of a Registered Nurse, including assistance with daily living activities, medication management, and personal care.
  • Assist patients with self-administered medications, including opening bottle caps, reading medication labels, and observing patients take their medications.
  • Take accurate temperature, pulse, and blood pressure measurements, and report any abnormal results to the supervisor.
  • Help patients maintain good personal hygiene, including bathing, grooming, skin care, and other activities.
  • Assist with patient feeding, including communicating basic principles of nutrition and observing and recording food and fluid intake.
  • Provide necessary skills to safely assist patients with mobility, exercises, positioning/turning, transfers, and ambulation, as per the Plan of Care and CenterWell Home Health policy.
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care.
  • Practice acceptable infection control principles and provide a clean, safe, and comfortable environment.

Requirements:

  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant (CNA) program within the last 24 months.
  • Must meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation are required.
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
