Job Responsibilities:
- Provide high-quality patient care to geriatric patients in a skilled nursing facility setting.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Administer medications, perform routine medical procedures, and monitor patient vital signs.
- Document patient progress and maintain accurate medical records.

Requirements:
- Current and valid SD (or compact state) LPN (Licensed Practical Nurse) license.
