Key Responsibilities- Develop trusting relationships with patients and their families as an end-of-life care expert
- Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient
- Continuously evaluate personal and professional performance to increase productivity and quality of care delivered
- Initiate and coordinate the plan of care, documenting problems, goals, interventions, and patient/family response to hospice care
- Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care daily
- Instruct and supervise the patient/family in self-care techniques when appropriate
