PDT Program Facilitator

The PDT Program Facilitator plays a crucial role in executing, teaching, and overseeing the Abili-IT initiative within the Employee Support Services (ESS).
This position is dedicated to supporting a designated group of individuals, fostering independence, personal development, and readiness for employment.
The facilitator ensures that employment integration, skills enhancement, and job exploration are central to every participant's experience. The role will involve a hybrid approach, combining activities at various locations.
KEY RESPONSIBILITIES
- Develop and implement a program schedule that encourages community engagement, self-help, social skills, and employment competencies based on a structured curriculum.
- Collaborate with ESS staff and individuals to execute their DORS plans within the supported employment framework, facilitating their integration into job skills development and exploration.
- Communicate with the DORS Case Manager and interdisciplinary team members regarding any behavioral or programmatic concerns that arise.
- Deliver services as specified in DORS Individual Plans, ensuring that goals are established, executed, and progress is monitored effectively.
- Track the progress of individual goals for each participant.
- Utilize positive behavioral management techniques and engage with individuals in a person-centered, age-appropriate manner.
- Maintain thorough, accurate, and timely documentation, including progress notes, daily logs, timesheets, and individual planning activities.
- Enhance individual self-esteem through positive interactions and by building trusting relationships.
- Connect with community resources to provide guest lectures that complement the Skills Training program and enrich the curriculum with volunteer opportunities and external educators.
- Attend and actively engage in mandatory training sessions, stakeholder meetings, and supervision discussions.
- Ensure that individuals receive appropriate training related to the programs and services offered.
- Uphold standard operating procedures within the Professional Development Training Program.
- Perform other related duties as assigned.

The incumbent will report to the Associate Director of Employee Support Services.
PHYSICAL QUALIFICATIONS
A flexible schedule is essential due to varying hours. The role requires the ability to stand or walk for 50% of the time, with the remaining time spent sitting or driving. The individual must be capable of lifting, carrying, pushing, and pulling up to 20 pounds, as well as performing tasks that involve stooping, kneeling, crouching, crawling, and reaching. These physical requirements are necessary to effectively supervise and assist individuals with disabilities.
QUALIFICATIONS
A Bachelor's degree in special education, social work, psychology, or a related field is required, along with experience working with individuals with disabilities. Certification in Cardiopulmonary Resuscitation (CPR) and First Aid is mandatory. Additional departmental training will be necessary to maintain this position. The candidate must demonstrate the ability to provide respectful support that meets the diverse needs of individuals served, along with prior teaching or training experience.
This role may require the use of a personal vehicle to transport individuals served. All transportation policies must be adhered to, and liability insurance must be current while transporting for or on behalf of the organization. Reimbursement for mileage will be provided according to the travel policy.
