Job Summary

We are seeking a skilled Hospice Certified Home Health Aide and CNA to join our team at Interim HealthCare Hospice - Sacramento, CA. As a compassionate caregiver, you will play a crucial role in ensuring our patients receive the highest quality of care in the comfort of their own homes.

Key Responsibilities
  • Provide personalized care to patients, including bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating, and nutritional intake.
  • Monitor patients' vital signs, intake/output, and blood glucose levels, and document any changes.
  • Work collaboratively with a team of hospice nurses to ensure a patient's care plan is followed.
  • Observe patients and notify supervisors of any issues requiring a change in care, medication, or medical equipment.
Requirements
  • High school diploma (or equivalent), active Home Health Aide certification, and active Nursing Assistant Certification in California.
  • CPR certification.
  • Covid-19 vaccination card.
  • Valid California driver's license, auto insurance, and transportation.
  • Compassionate nature, good communication skills, and ability to lift up to 50 lbs.
