Our Clinically Integrated Network is designed to provide enhanced care to Medicare Advantage, Quality plan, and ACO members by delivering high-quality, coordinated healthcare that produces superior outcomes in a cost-effective manner.
The primary role of our Primary Care Physician (PCP) is to lead clinical teams in developing plans of care, providing patient/family/significant other counseling, ordering laboratory and diagnostic studies, prescribing medications, and referring patients for consultations when necessary.
As a PCP, you'll have access to a robust transitional care and high-risk outreach program, sophisticated electronic medical records, and advanced analytical data analysis to identify patients at risk for poor outcomes.
You'll also participate in patient care rounds, communicate patient management strategies to team members, collaborate with multidisciplinary teams, and recognize situations requiring immediate attention and initiate life-saving procedures when necessary.

This position requires a Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.), or M.B.B.S., board certification or eligibility in Family or Internal Medicine, an active, unrestricted medical license in Louisiana, and a valid DEA license.
