Position Overview:
We are in search of a dedicated and experienced RN Case Manager to join our healthcare team on a PRN basis. This role requires availability on weekdays, weekends, and holidays, with an orientation period of approximately 2-4 shifts over 3 weeks during weekdays.
Key Responsibilities:
As an RN Case Manager, you will be responsible for:
- Coordinating services throughout the continuum of care to ensure quality patient care while effectively utilizing resources.
- Conducting concurrent clinical reviews to assess the appropriateness of admissions and the ongoing need for care in the acute hospital setting.
- Facilitating collaboration and communication among patients, families, and the healthcare team regarding the patient's care plan and post-discharge needs.
- Performing telephonic clinical reviews with insurance claims personnel to support reimbursement for patient care services.
- Identifying quality and risk issues and reporting them to the appropriate departments.
Qualifications:
To be considered for this role, you will need:
- Current licensure as a Registered Nurse in the State of Colorado or an active multistate Registered Nurse license.
- Current BLS certification from the American Heart Association or American Red Cross. Case Management or CPHQ Certification is strongly preferred.
- A degree from an Accredited School of Nursing; a BSN is desirable.
- A minimum of 2 years of clinical nursing experience in an acute care setting, with skills in assessing, planning, and managing patient care delivery.
- Prior experience in utilization, quality, discharge planning, and/or case management is highly preferred.
