Key Responsibilities
  • Perform comprehensive diagnostic assessments to identify patient needs and develop effective treatment plans.
  • Provide patient visits as scheduled, ensuring timely and compassionate care.
  • Ensure treatment plans are up-to-date for each client, reflecting their evolving needs and progress.
  • Complete comprehensive and succinct progress notes, documenting patient interactions and outcomes.
  • Communicate and follow up as needed regarding the client's status, ensuring seamless care coordination.
  • Collaborate with case management and psychiatric services teams to provide holistic patient care.
Requirements
  • Bachelor's degree in Social Work or a related field, with one year of therapy experience, or a Master's degree in Social Work or Professional Counseling.
  • Social Work/Professional Counseling License or equivalent: LSW, LISW, LISW-S, LPC, LPCC, LPCC-S.
  • Experience in diagnosis, therapy, and counseling is mandatory.
  • Experience in EAP or integrated health practice is preferred.
  • Experience in providing evaluation and brief therapy interventions is preferred.
  • Valid driver's license, car insurance, and reliable transportation.
  • Willingness to travel throughout assigned service areas.
