Key Responsibilities:
- RAI Process Coordination: Complete and coordinate the RAI process to ensure accurate and timely assessments.
- MDS Assessment Submission: Ensure timely submission of MDS assessments to meet regulatory requirements.
- Interdisciplinary Collaboration: Collaborate with healthcare team members to develop individualized care plans.
- Care Plan Development: Develop and implement individualized care plans that meet patients' needs.
- Regulatory Compliance: Stay informed about regulatory changes and ensure compliance with Medicare and Medicaid regulations.
- Staff Education: Provide education and training to staff members on RAI process and MDS completion.
Qualifications:
- Current RN License: Current RN license in New York.
- MDS Coordinator Experience: Minimum 1 year experience as an MDS Coordinator.
- RAI Process Knowledge: In-depth knowledge of RAI process and MDS completion.
- Regulatory Knowledge: Strong understanding of Medicare and Medicaid regulations.
- Organizational and Communication Skills: Excellent organizational and communication skills.
- Technical Skills: Proficiency in Sigma Care.
