Key Responsibilities
  • Conduct ongoing participant assessments and monitoring to ensure their physical and emotional well-being.
  • Provide assistance with daily activities, including toileting, grooming, and meals, to promote independence and dignity.
  • Aid in maintaining physical activity and exercise routines to enhance overall health and quality of life.
  • Identify safety hazards and respond to emergencies to ensure a safe environment for participants.
  • Assist with recreational activities, meal service, and housekeeping to create a comfortable and engaging living space.
  • Report any changes in participant's condition to center staff to ensure timely interventions.
  • Escort participants to appointments and assist with transportation to ensure access to necessary healthcare services.
Requirements
  • High School Diploma or equivalent.
  • Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization upon hiring.
  • Preferred experience in long-term care or geriatric programs.
  • Strong communication and organizational skills.
  • Valid driver's license, insurance, and use of personal vehicle.
  • Ability to work independently and adapt to change.
