Compensation: Starting at $34.00 per hour

Position Summary:
The Case Management Nurse operates with a degree of autonomy, utilizing clinical expertise to assist clients with health-related and rehabilitation activities. We encourage applications from retired case management nurses, recent nursing graduates, and those seeking a flexible daytime schedule.
Key Responsibilities:
- Conduct 1-2 home visits daily.
- Engage in required in-person meetings and assessments.
- Perform clinical eligibility evaluations for adult foster care participants.
- Complete necessary prior authorizations.
- Work independently while ensuring compliance with program and contractual standards.
- Support program organization, expansion, and staff training for implementation in various areas.
Qualifications:
- Must possess a valid MA RN license.
- Preferred: Minimum of 2 years experience in human services.
- Valid driver's license with a good driving record and reliable transportation.
- Understanding and respect for diverse populations.
- Strong communication, interpersonal, teamwork, and listening abilities.
- Proven reliability and dependability.
