Job Overview

We are seeking a highly skilled Clinical Social Worker to join our team at Suburban Hospital. As a Care Coordinator, you will play a vital role in ensuring optimal clinical quality and financial outcomes for our patients.

Key Responsibilities:

  • Coordinate patient care services and resources to ensure seamless transitions and effective communication among healthcare team members.
  • Utilize screening mechanisms to identify patients at risk for lengthy inpatient stays or complex clinical issues.
  • Collaborate with the Case Management Coordinator and other healthcare professionals to develop comprehensive care plans.
  • Document patient information and progress according to established guidelines.

Requirements

To be successful in this role, you will need:

  • A Bachelor's Degree in Behavioral Health, Social Work, or a related field.
  • Experience as a social worker in a hospital environment, preferably with experience in case management and coordination of care.
  • Social Work licensure required or LSWA.
  • Current LCSW certification preferred.
