St. Mary's Health Care System is seeking a skilled LPN to join our team in a Long Term Care Facility.

**Job Summary:**

We are looking for a dedicated and compassionate LPN to provide direct personal care to residents in all activities of daily living.

**Responsibilities:**

* Provide direct personal care to residents in all activities of daily living as prescribed by the philosophies, standards, policies and procedures of the facility.
* Work in a fast-paced environment, prioritizing tasks and managing time effectively.
* Collaborate with the healthcare team to ensure high-quality care and services.
* Maintain accurate and detailed records of resident care and services.
* Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

**Requirements:**

* Current LPN licensure in the state of employment.
* Previous experience in an assisted living facility and/or with individuals with dementia or Alzheimer's preferred but not required.
* Strong communication and interpersonal skills.
* Ability to work effectively in a team environment.
* Commitment to providing high-quality care and services to residents.
