Job Summary

Apace is seeking a highly skilled and compassionate Support Professional to join our team. As a key member of our staff, you will play a vital role in providing training and support to individuals with disabilities in their homes and community locations.

Key Responsibilities
  • Develop and implement personalized training programs to enhance the skills and abilities of individuals with disabilities.
  • Provide ongoing support and guidance to clients in their daily lives, including assistance with personal care, meal preparation, and household tasks.
  • Collaborate with clients, family members, and other stakeholders to identify and achieve individual goals and objectives.
  • Conduct regular assessments and evaluations to ensure that clients are receiving the most effective and efficient support possible.
  • Maintain accurate and detailed records of client progress and services provided.
  • Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments.
Requirements
  • High school diploma or equivalent required; degree in a related field (e.g. social work, special education) preferred.
  • Minimum one year of experience working with individuals with disabilities; experience in a similar role or industry a plus.
  • Excellent communication and interpersonal skills; ability to work effectively with diverse populations and stakeholders.
  • Ability to lift up to 50 pounds and perform physical tasks as required by the job.
  • Valid driver's license and reliable transportation required.
