About the Position

  • Responsible for providing physical therapy services to adult and geriatric patients in Senior Living Communities and Independent Living Communities.
  • Collaborate with nurses and other healthcare professionals to develop and implement comprehensive care plans.
  • Conduct thorough assessments and create personalized treatment plans to achieve optimal patient outcomes.
  • Maintain accurate and detailed records of patient progress and provide regular updates to healthcare teams.

Requirements and Qualifications

  • Current Pennsylvania Physical Therapist - PT license.
  • One year of recent work experience as a Physical Therapist - PT.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Solid computer skills and experience with electronic medical records (EMR).
