Job Summary

Wexford Health Sources is seeking a dedicated Licensed Practical Nurse to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the overall success of our organization.

Key Responsibilities
  • Assist in the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation.
  • Direct and guide patient teaching activities under the supervision of Registered Nurses.
  • Utilize electronic medical record systems to document patient information and track progress.
  • Collaborate with healthcare professionals to develop and implement individualized treatment plans.
  • Provide emotional support and education to patients and their families.

Requirements

To be successful in this role, you will need to have a current LPN license in the state of Illinois and CPR certification. Prior experience with electronic medical record systems is also preferred.
