Position Overview:
Freedom Home Health Care is seeking a dedicated and experienced Home Health Nurse to oversee a team of nursing professionals responsible for a caseload of 35-40 clients.


Key Responsibilities:
The Case Manager will take charge of creating comprehensive patient care plans, employing a holistic approach to ensure optimal care within the home environment.

This role involves coordinating care during admissions, transfers, and discharges, as well as conducting routine visits.


Home Visits:
As part of the responsibilities, the Case Manager will perform home visits to establish medication regimens, provide wound care, and deliver patient education in collaboration with visiting nurses, thereby enhancing the quality of care.


Compensation and Benefits:
Freedom Home Health Care offers a competitive base salary based on minimum acuity, along with bonus per diem compensation to acknowledge staff members who demonstrate proficiency in navigating electronic health records, thereby promoting efficiency.

We provide a comprehensive benefits package, including Health, Dental, STD/LTD Insurance, and a 401K plan with corporate matching. Our centralized service area is designed to reduce travel time between visits, complemented by mileage reimbursement.
