We are seeking a compassionate and organized LPN/LVN to join our team at Bear Canyon Rhb Ctr in Albuquerque, New Mexico. As a Licensed Practical Nurse/Licensed Vocational Nurse, you will be responsible for providing exceptional care to our patients in a long-term care setting.
Key Responsibilities- Deliver high-quality patient care in a long-term care environment
- Collaborate with the healthcare team to develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and report any changes to the healthcare team
- Provide emotional support and education to patients and their families
- Current New Mexico license as a LPN/LVN
- At least two years of recent experience as a LPN/LVN in long-term care
- Current BLS and/or ACLS and/or Specialty Certifications
- 2 current Clinical References
- Physical
- TB Records
- Current - Tdap/Flu vaccinations
