Job Summary

Amergis is seeking a skilled Long Term Care LPN to join our team. As a Long Term Care LPN, you will be responsible for delivering high-quality patient care in a Long Term Care/Assisted Living Facility.

Key Responsibilities
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility.
  • Perform focused interviews to identify specific patient needs.
  • Assess signs and symptoms indicating physiologic and psychological changes in the patient's condition.
  • Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
  • Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
  • Identify and prioritize actions based on patient care requirements.
  • Monitor consistency between patient plan of care and actions of medical staff.
  • Perform interventions according to identified priorities, plan of care, and facility policies and patient outcome standards.
  • Provide a safe, comfortable, and therapeutic patient environment.
  • Initiate and evaluate patient and/or family education.
  • Revised the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
  • Document patient's response to nursing and other interventions.
  • Use clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit, including establishing priorities.
  • Monitor safety and effectiveness of the environment and equipment.
  • Participate in activities to facilitate good interpersonal communications with the chosen facility.
  • Support professional practice by observing the following: uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patient's Bill of Rights; and 6) Amergis Healthcare, nursing and client hospital policies.
Requirements
  • Current Licensed Practical Nurse License for the state in which the nurse practices.
  • Ability to effectively elicit/provide information to and from appropriate individuals via strong spoken, written, listening, and nonverbal communication skills.
  • Current Health Certificate (if applicable).
  • Current PPD or Chest X-Ray.
  • One year prior professional nursing experience preferred.
  • Current BLS card.
  • Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater.
  • Successful completion of Background Screening Process.
