The successful candidate will demonstrate proficiency as a general practitioner in Occupational Therapy, utilizing diverse evaluation and treatment methodologies, alongside exceptional communication skills.
As an LMSW/LMGW Inpatient Case Manager, you will be an integral part of the Case Management Team, facilitating the provision of high-quality, cost-effective, and patient-centered care throughout the continuum of care, from pre-admission to post-discharge. Your role will involve ensuring that care plans are tailored to meet the unique needs of each patient, monitoring the services provided during their hospital stay, and promoting effective case management practices to optimize clinical and resource outcomes.
Key Responsibilities:
- Collaborate with a multidisciplinary healthcare team to formulate and coordinate comprehensive care plans. This includes setting both long- and short-term goals, identifying necessary services, and involving family members or caregivers in the planning process. Timely referrals to community agencies will also be part of your responsibilities.
- Engage in daily communication with direct caregivers and the case management triad to discuss patient and family responses to care plans, identify potential issues, and manage discharge planning and payer concerns.
- Ensure accurate and timely documentation of case management activities, adhering to all relevant policies and confidentiality standards. Demonstrate an understanding of abuse and neglect dynamics, coordinating with appropriate agencies as necessary.
- Maintain a comprehensive knowledge of community resources and referral protocols to support the patient population effectively. Facilitate communication between patients, families, and the healthcare team, addressing social, cultural, emotional, and economic challenges that may affect patient recovery.
- Commit to ongoing professional development in managed care, care management, and clinical practices. Organize your workload effectively, prioritizing tasks based on patient needs and institutional policies.
- Master's degree in social work from an accredited institution.
- Valid Social Worker license in the relevant jurisdiction.
- Preferred experience in a hospital social work setting.
- Preferred experience in care or case management.
- Certification as a Certified Case Manager (CCM) is preferred.
