About the Role

We are seeking a skilled Registered Nurse to join our Home Health team at Providence In-Home Services in Anchorage. As a Home Health RN, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and improving health outcomes.

Responsibilities
  • Assess patients' physical and emotional needs, developing personalized care plans to address their unique requirements.
  • Deliver skilled nursing care, including wound care, medication management, and education on disease management and self-care.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordinate services with other healthcare providers.
  • Document patient progress, identifying areas for improvement and implementing changes to optimize care.
  • Provide education and support to patients and their families, empowering them to manage their health and make informed decisions.
Requirements
  • Graduate of an accredited/approved school of Nursing.
  • Alaska Registered Nurse License.
  • National Provider BLS - American Heart Association.
  • Valid driver's license and reliable transportation.
Preferred Qualifications
  • One year of experience as a Registered Nurse in an acute care hospital or home health setting.
