Key Responsibilities- Assessment and Treatment Planning: Conduct comprehensive assessments of patients' psychosocial needs and develop treatment plans in collaboration with licensed clinical supervisors.
- Behavioral Health Treatment and Education: Provide effective behavioral health treatment and education to patients, including assessment, psychotherapeutic counseling, and therapy.
- Documentation and Record-Keeping: Maintain accurate and timely documentation of patient interactions and treatment plans in accordance with department and hospital policies.
- Collaboration and Communication: Work collaboratively with patients, families, and other healthcare professionals to develop comprehensive discharge plans and ensure seamless transitions of care.
- Case Management and Referrals: Identify and address social determinants of health, provide information, education, and referrals as needed, and facilitate connections to community resources.
- Emergency Response and Crisis Intervention: Assist in responding to emergencies and provide crisis intervention services as needed.
- Professional Development and Supervision: Participate in ongoing professional development, receive regular supervision, and maintain licensure and certification requirements.
- Education: Master's degree in Social Work (MSW), Master's degree in Counseling, or related field.
- Experience: 0-3 years of total work experience in Social Work or related field.
- Licensure and Certification: Associate Counselor License or NJ Licensed Social Worker or Marriage and Family Therapy, Associate Counselor. Handle with Care certification required.
