About the JobYou will join an experienced team that values strong communication and teamwork abilities. Your responsibilities will include:
- Completing admissions, social services evaluations, providing for social services or referrals, and maintaining communication with residents and families.
- Assisting residents with financial and legal matters, such as applying for pensions, referrals to lawyers, and referrals to funeral homes for preplanning arrangements;
- Providing discharge planning services, including helping to place residents on waiting lists for community congregate living, arranging intake for home care services, assisting with transfer arrangements to other facilities, and working with outside agencies toward safe discharges;
- Providing or arranging needed counseling services and psychiatric medication management as indicated.
We require a current social work licensure in the state, a Bachelor's degree in social work, and one year of supervised social work experience in a healthcare setting.
