Outpatient Therapist

Job Summary:
Crossroads Health is seeking a compassionate and skilled Outpatient Therapist to join our team. As a licensed mental health professional, you will provide individual and family therapy to clients in our Mentor, Lake County office.
Key Responsibilities:
- Conduct diagnostic assessments and develop client-centered treatment plans
- Provide outpatient individual, group, and family therapy to children, adolescents, and families
- Maintain accurate and detailed clinical records
- Collaborate with multidisciplinary teams to ensure comprehensive care
- Participate in outcome tracking and quality improvement initiatives
- Respond to emergencies and provide crisis intervention as needed
Requirements:
- Bachelor's degree in a related field with 3 years of experience, or Master's degree in Counseling or Social Work
- Current licensure as a Licensed Professional Counselor (LPC), Licensed Social Worker (LSW), or Marriage and Family Therapist (MFT)
- 2+ years of experience in the mental health field, preferably in an outpatient setting
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced, high-stress environment
- Strong crisis management and de-escalation skills
