Job SummaryWe are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team. As a key member of our team, you will conduct in-home assessments, perform evidence-based practice screenings, and identify diagnoses to inform care management and active medical management.
Responsibilities- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses to inform care management and active medical management.
- Communicate findings to inform member's primary care physician of potential gaps in care.
- Identify urgent and emergent situations and intervene appropriately.
- Educate members on disease processes, medications, and compliance.
- Active NP or PA license or ability to obtain by start date.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and sitting, standing, and kneeling to perform physical assessments.
- Reliable transportation to complete home visit assessments.
- Fluency in Spanish.
- 1+ years of clinical experience in their highest level of education, clinical setting.
- May be requested to obtain additional licensure in other geographic areas.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, beyond your assigned region, up to 5% of the time, with advanced notice and eligible for additional compensation incentives.

New York Residents Only: The salary range for this role is $88,000 to $173,200 annually.
