Job Overview

We are seeking a highly motivated and compassionate Case Manager to join our team at Friends of Children with Special Needs. As a key member of our Supported Living Services (SLS), you will provide exceptional care and support to individuals with developmental disabilities, empowering them to lead fulfilling lives.

About the Role

This is a full-time exempt position that offers a competitive salary range of $64,480-$74,880 per year, depending on experience. The successful candidate will be responsible for providing comprehensive case management services, including:

  • Coordinating resources and obtaining living arrangements and public benefits for participants
  • Scheduling staffing and supervising living service trainers to enhance quality of service delivery
  • Conducting ISP meetings and building relationships with stakeholders
  • Acting as the designated emergency leader when the supervisor is not available
Responsibilities

The ideal candidate will possess excellent behavioral management skills, people skills, and problem-solving skills. They will be proficient in using Microsoft Office applications, Google Drive, and Presentation applications, and have excellent oral and written communication skills. The responsibilities include:

  • Providing initial orientation and shadowing for new hires, reviewing their performances, and providing constructive feedback
  • Conducting ongoing training, facilitating bi-weekly/monthly apartment meetings, and providing close supervision to promote staff's proficiency
  • Supervising, mentoring, and evaluating staff's performance, PCT implementations, and documentations to ensure quality, accuracy, and compliance with program standards
  • Carrying rotated 24-hour emergency phone and providing support and crisis intervention
Requirements

To be successful in this role, you will need:

  • An associate's degree in a human services field, with a bachelor's degree or higher education preferred
  • A minimum of two years' experience in working in the developmentally disabled field
  • Excellent knowledge of developmental disabilities, CCL regulations, RC system, Lanternman Act, DDS, and behavioral management skills
