Social Services Coordinator

We are seeking a highly motivated and compassionate Case Manager to join our team at Friends of Children with Special Needs. As a key member of our Supported Living Services (SLS), you will provide exceptional care and support to individuals with developmental disabilities, empowering them to lead fulfilling lives.
About the RoleThis is a full-time exempt position that offers a competitive salary range of $64,480-$74,880 per year, depending on experience. The successful candidate will be responsible for providing comprehensive case management services, including:
- Coordinating resources and obtaining living arrangements and public benefits for participants
- Scheduling staffing and supervising living service trainers to enhance quality of service delivery
- Conducting ISP meetings and building relationships with stakeholders
- Acting as the designated emergency leader when the supervisor is not available
The ideal candidate will possess excellent behavioral management skills, people skills, and problem-solving skills. They will be proficient in using Microsoft Office applications, Google Drive, and Presentation applications, and have excellent oral and written communication skills. The responsibilities include:
- Providing initial orientation and shadowing for new hires, reviewing their performances, and providing constructive feedback
- Conducting ongoing training, facilitating bi-weekly/monthly apartment meetings, and providing close supervision to promote staff's proficiency
- Supervising, mentoring, and evaluating staff's performance, PCT implementations, and documentations to ensure quality, accuracy, and compliance with program standards
- Carrying rotated 24-hour emergency phone and providing support and crisis intervention
To be successful in this role, you will need:
- An associate's degree in a human services field, with a bachelor's degree or higher education preferred
- A minimum of two years' experience in working in the developmentally disabled field
- Excellent knowledge of developmental disabilities, CCL regulations, RC system, Lanternman Act, DDS, and behavioral management skills
