About the Role

As a Healthcare Professional - Patient Connect, you will play a critical role in ensuring a smooth transition from rehab to home for patients who have recently been hospitalized. Your primary responsibilities will include:

  • Performing comprehensive assessments and documenting findings in a concise and compliant manner.
  • Ensuring accurate and timely documentation of diagnoses, medications, and other relevant information.
  • Communicating effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participating in quality initiatives and contributing to the development of new and improved methods and approaches to concerns and issues.
Requirements

To be successful in this role, you will need:

  • A graduate degree in Nursing (MSN) or a related field, with certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
  • Active and unrestricted licensure in the state where you reside.
  • Current DEA licensure or ability to obtain post-hire, per state regulations.
  • Access to reliable transportation and ability to travel to client and/or patient sites.
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions.
Preferred Qualifications

We welcome candidates with experience in Long Term Care, Geriatrics, and Chronic Illness. If you have a strong understanding of Advanced Illness and end-of-life discussions, and are proficient in computer skills, including electronic medical records, we encourage you to apply.
