Responsibilities- Assess patients' physical and psychosocial status, administering medication and documenting vital signs.
- Rend and supervise care of clients on the unit, ensuring effective communication with staff and providers.
- Monitor medication and consult with prescribers to ensure safe and effective treatment.
- Provide health education and coordinate referrals as prescribed by providers.
- Participate in patient evaluations and progress monitoring with team members.
- Promote wellness and medically related psychoeducational groups to individuals.
- Record client information within electronic medical records daily.
- Assist RNs on a daily basis and offer nursing consult on emergency service evaluations.
- Work with clients to coordinate care with their PCP, psychiatrist, and other professionals.
- Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
- Satisfactory CORI background check.
- Minimum 2 years' experience in medical nursing, with knowledge of medical, substance abuse, and mental health nursing.
- Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
- Demonstrates ability to communicate effectively and professionally with clients, staff, and providers.
The Licensed Practical Nurse will work in a fast-paced healthcare environment, requiring the ability to move about the space periodically during the shift, stoop, crouch, kneel, or crawl for various reasons, and hear, understand, and distinguish speech and/or other sounds.
