POSITION SUMMARY

The Street Medicine Advanced Practice Provider plays a crucial role in the Street Medicine initiative. This position addresses the dynamic healthcare requirements of homeless and underserved communities, providing a unique opportunity to make a significant impact directly within the community and clinical settings.

KEY RESPONSIBILITIES

  • Acts as a primary healthcare provider, delivering compassionate and thorough medical services in unconventional environments, including community streets, encampments, and temporary shelters.
  • Adjusts clinical practices to align with the diverse conditions and resources present in both clinical and street-based settings.
  • Collaborates with a multidisciplinary street medicine team to innovate and enhance the service delivery framework.
  • Leads educational programs for team members, promoting a culture of ongoing learning and flexibility.
  • Embraces the unpredictable aspects of street medicine, utilizing creativity and resourcefulness to navigate challenges.
  • Engages in case management, demonstrating responsiveness to unforeseen patient needs and circumstances.
  • Provides urgent care for unsheltered homeless individuals with acute or chronic medical and/or psychiatric issues, including medication management.

Direct Patient Care Responsibilities:

  • Conducts comprehensive physical assessments, including reviewing patient medical histories.
  • Orders, administers, and interprets diagnostic tests, such as x-rays, electrocardiograms, and laboratory analyses.
  • Performs therapeutic interventions, including injections, immunizations, suturing, and wound care, while managing infections.
  • Instructs and advises patients on adherence to prescribed treatment plans, developmental milestones, family planning, emotional well-being, and health maintenance.
  • Enhances patient experiences through courteous and accurate service.
  • Delivers age-appropriate care tailored to individual patient needs.

Team Collaboration:

  • Shares responsibility for overall patient health outcomes, coordinating with care teams.
  • Instills confidence in our care and facilities among patients, colleagues, and stakeholders.
  • Responds promptly and compassionately to the needs of patients and staff.
  • Maintains professional relationships with all levels of personnel, clients, and the public.
  • Contributes to achieving departmental goals and objectives.

Clinical Administration Duties:

  • Develops and implements patient management strategies, documenting progress and ensuring continuity of care.
  • Operates in accordance with practice protocols and under the supervision of designated medical professionals, as outlined in the Supervisory Agreement.

EDUCATION AND EXPERIENCE REQUIREMENTS

  • Master's degree from an accredited physician assistant or nurse practitioner program.
  • Current California Physician Assistant or Nurse Practitioner license.
  • Active California DEA license.
  • Current BLS certification from an approved provider required upon hire.
  • Preferred experience in prescribing buprenorphine for medication-assisted treatment.
  • Preferred experience in psychiatric medication management.
  • Bilingual proficiency in English and Spanish is preferred.

ADDITIONAL QUALIFICATIONS

  • Strong verbal and written communication skills, including excellent composition, typing, and proofreading abilities.
  • Ability to interpret various forms of instructions, including written, oral, and diagrammatic.
  • Proficient in word processing, spreadsheets, and electronic medical records systems.
  • Ability to serve marginalized populations effectively.
  • Capability to improvise and solve problems in non-standard environments.
  • Ability to navigate ambiguity and identify best practices to devise effective solutions.
  • Proficient in managing multiple tasks simultaneously.
  • Excellent organizational and planning skills.
  • Ability to work collaboratively as part of a team and independently.
  • Ability to handle confidential information with professionalism and ethics.

PHYSICAL REQUIREMENTS

  • Ability to lift and carry up to 20 lbs.
  • Ability to stand for extended periods.

COMPENSATION:

  • Annual salary range: $154,000 - $209,000, based on experience.
