Case Manager III

We are seeking a highly motivated and experienced Case Manager III to join our team at the City of Norfolk, VA.
Job Summary:
The Case Manager III will be responsible for providing case management services to individuals with intellectual and developmental disabilities, including linking, monitoring, and advocating for clients. This is a challenging and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:
- Completes initial and annual assessments of individuals to plan services and supports
- Develops individual support plans in collaboration with the individual and/or family
- Provides linkage, coordination, and monitoring for services, including Medicaid Waiver Services
- Maintains chart documentation in compliance with agency guidelines and state regulations
- Ensures that documentation substantiates billing for services
- Complies with all State Case Management Licensure regulations, HIPAA, and Human Rights regulations
- Conducts face-to-face visits at a minimum of every 90 days and completes monthly activity related to the individual service plan
- Monitors progress, service delivery, and individual satisfaction by providing monthly service to all individuals on assigned caseload
- Identifies and accesses resources to meet identified needs and assist individuals in procuring and maintaining resources and entitlements
Requirements:
- Bachelor's degree in a human services field
- Two years of experience in a human services field related to the areas of assignment
- Valid Driver's License may be required depending on assignment
- Certification in basic first aid and CPR, depending on area of assignment
