**Job Summary**
Greenlife Healthcare Staffing is seeking a highly skilled Clinical Social Worker Health Care Facility Surveyor to join our team. As a surveyor, you will be responsible for accrediting and certifying facilities that serve residents of long-term care facilities.
**Responsibilities**
* Conduct surveys of long-term care facilities to ensure compliance with CMS regulations
* Evaluate the quality of care provided to residents
* Identify areas for improvement and provide recommendations for corrective action
* Collaborate with facility staff to implement changes and improve quality of care
**Requirements**
* Master's degree in Social Work
* Successful completion of CMS's Long-Term Care Basic Training and SMQT certification
* At least two years of recent experience working as a surveyor accrediting or certifying facilities
* Current licensure to practice as an LCSW
