Responsibilities
  • Conduct thorough psychosocial assessments to identify patient and family needs, goals, and interventions.

  • Provide emotional support and counseling to patients and their families, addressing their unique needs and concerns.
  • Collaborate with other healthcare professionals to ensure continuity of care and effective communication.
  • Document patient progress and outcomes, maintaining accurate and detailed records.
Requirements
  • Master's Degree in Social Work from a CSWE-accredited school.
  • Licensure as required by the state in which the hospice is located.
  • At least one year of experience as a Social Worker in a healthcare and/or hospice setting, with three to five years preferred.
  • Knowledge of terminally ill patients and their families, as well as understanding of hospice and the psychosocial dynamics of illness, loss, and death.
