Pharmaceutical Care Specialist

Position Overview:
The Clinical Pharmacist will deliver pharmaceutical care in alignment with Tohono O'odham Nation Health Care Pharmacy standards, federal regulations, and best practices.
Key Responsibilities:
- Execute outpatient pharmacy operations encompassing a wide range of pharmaceuticals, including controlled substances and investigational drugs.
- Process and validate new prescription orders and refill requests.
- Review and confirm medications prepared by pharmacy technicians for accurate dispensing.
- Provide exceptional medication-related customer service and collaborate with other departments to ensure optimal patient care.
- Maintain up-to-date knowledge of prescription medications and stay informed about medication safety issues.
- Ensure the accuracy of medication records and utilize electronic health records for documentation and drug allergy management.
- Assess prescriptions and patient medication profiles to determine the appropriateness of prescribed therapies.
- Compound medications as required, determining formulations, dosages, and quantities.
- Conduct patient education regarding medication administration, potential reactions, and treatment plans.
- Manage inventory records for controlled substances and oversee medication procurement.
- Complete necessary documentation in compliance with federal and service unit policies.
- Conduct inspections of drug storage areas to ensure proper handling and accountability.
- Collaborate with physicians on complex cases, advising on drug therapies and treatment options.
- Evaluate drug regimens for efficiency and cost-effectiveness.
- Document medication errors and unusual incidents as required.
- Provide inpatient pharmacy services, including the preparation of sterile products and intravenous medications.
- Review physician orders and patient records to prevent adverse drug interactions.
- Ensure proper labeling, handling, and storage of all pharmaceutical products.
- Guide and support junior staff, ensuring adherence to professional standards and regulations.
- Conduct training sessions for staff on advancements in drug therapy.
- Assist in training pharmacy students and personnel in medication preparation and administration.
- Offer specialized services for chronic illness management, including diabetes and hypertension.
- Represent the pharmacy department in various committees and meetings.
- Support clinical research initiatives as necessary.
- Provide pharmacy and drug information services to eligible individuals in various community settings.
- May work alternative schedules to ensure service continuity.
- Contribute to a collaborative team environment.
- Perform additional duties as assigned to meet pharmacy goals.
- Understand and respect the cultural needs of the tribe.
- Expertise in clinical pharmacy principles and practices.
- Understanding of federal pharmacy regulations.
- Knowledge of drug properties, therapeutic actions, and biopharmaceutical science.
- Experience in planning and evaluating drug therapy in clinical settings.
- Familiarity with resource materials and their applications.
- Strong communication skills for patient education on medication use.
- Ability to collaborate with healthcare providers for comprehensive patient care.
- Competence in documenting patient information accurately.
- Ability to work independently and make authoritative pharmacy decisions.
- Experience supervising support staff in pharmacy operations.
- Ability to distinguish basic colors.
Minimum Qualifications:
- Doctoral degree in Pharmacy from an accredited institution.
Licenses and Certifications:
- Active pharmacy license in at least one state or U.S. Territory.
- Completion of a clinical pharmacy residency is preferred.
- Immunization against Measles and Rubella is required, along with an annual influenza vaccination.
- Criminal background and National FBI fingerprint check required upon hire.
- Valid driver's license may be required.
- Fluency in both the Tohono O'odham language and English may be necessary based on departmental needs.
