Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our interdisciplinary team at Hospice of Northwest Ohio. As an LPN, you will provide high-quality patient care, work collaboratively with our team, and contribute to the delivery of exceptional hospice services.

Responsibilities
  • Perform direct nursing care, nursing interventions, and prescribed treatments based on established plans of care and hospice policies and procedures.
  • Observe and report patients' physical, emotional, and spiritual needs, as well as changes in their condition and response to treatment plans, to the RN.
  • Administer medications according to physician orders and hospice policies and procedures, and obtain and process physician orders.
  • Provide education and information to patients, families, and nursing home facility staff (if applicable) related to the patient's plan of care and needs.
  • Complete clinical documentation according to agency standards.
  • Participate in the agency's Quality Improvement program.
  • Attend staff meetings and complete all mandatory training.
  • Maintain confidentiality of patient information and disclose information only to authorized personnel.
Requirements
  • Graduate of an accredited school of practical nursing with two years of clinical experience, preferably in a nursing home environment or hospice.
  • Current licensure as an LPN through the Ohio Board of Nursing, with the ability to obtain a license in Michigan.
  • Verification of successful completion of a medication course.
  • Experience charting in an EMR system, preferably Net Smart.
  • Strong verbal and written communication skills, with the ability to collaborate with the interdisciplinary team.
  • Organizational, time management, and prioritization skills.
  • Licensed driver with an automobile in good working condition and insurance meeting state and agency requirements.
  • CPR certification according to American Heart Association standards.
