Job Summary: We are seeking an Independent Optometrist to lease space in our Walmart stores and provide high-quality eye and vision care services to our customers.

Key Responsibilities:

  • Establish your own days of practice and hours of operation
  • Control your patient schedule and set your own fee schedule
  • Keep 100% of exam/professional fees
  • Option to opt into equipment/instrumentation and administrative support services (if state allows)

Requirements:

  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
