Position Summary:

The Certified Nursing Assistant/Caregiver provides medical and physical care to residents, enabling them to function at their highest possible level. This role involves advocating for residents' needs, promoting their psychosocial well-being, and collaborating with the household team to meet social service needs.

Essential Duties and Responsibilities:
  1. Attend to residents' personal hygiene and care needs, including grooming, hygiene, dining, mobility, elimination, psychosocial, and safety needs.
  2. Organize necessary supplies and support residents in self-care, providing care as needed for those unable to care for themselves.
  3. Follow universal precautions, proper infection control, sanitation, and safety standards in all work and activities.
  4. Provide restorative therapy under the leadership of therapy professionals and in accordance with the resident's functional maintenance plan.
  5. Observe residents closely, identifying changing needs and conditions, and report immediately to nursing.
  6. Participate actively in the resident's plan of care, serving as their closest advocate in directing their care and honoring their preferences.
  7. Work collaboratively with the household team to meet residents' needs and desires.
  8. Continually seek opportunities to meet residents' psychosocial needs through care planned and spontaneous actions.
  9. Plan and organize social activities for individual residents, small groups, and the household, making necessary arrangements to ensure success.
  10. Maintain equipment, games, and supplies for use in supporting resident and household activity choices.
  11. Maintain clean spaces in the household, including resident rooms, common spaces, support spaces, and equipment.
  12. Maintain a supply of materials to meet resident needs in the household.
  13. Support all aspects of the household environment, including pets, plants, children, visitors, and guests, facilitating planned and spontaneous needs and activities.
  14. Coordinate work with the household homemaker to ensure personal laundry needs are met within staffing resources.
Education and/or Experience:

Graduate of a state-approved program for nursing assistants, competency evaluated, tested, and registered with the state. High school graduate or GED preferred, but not required. 2+ years in a Skilled Nursing Facility and dementia experience preferred.
