We are seeking a skilled Licensed Practical Nurse to join our team at Caresense Home Care. As a home health LPN, you will play a vital role in delivering high-quality patient care and promoting quality of life for our clients.

Job Summary

As a Licensed Practical Nurse with Caresense, you will administer skilled nursing care to patients in their place of residence under the direction and supervision of a Registered Nurse. Your primary responsibility will be to assess patient needs, develop and implement individualized care plans, and provide direct patient care services.

You will work closely with our interdisciplinary team to ensure seamless delivery of care and maintain effective communication with patients, families, and healthcare professionals.

Responsibilities
  • Administer skilled nursing care to patients on an intermittent basis in their place of residence.
  • Participate in the planning and coordination of total patient care in conjunction with the RN and physician's plan of care.
  • Accurately report and record patient condition and care, including signs and symptoms indicative of change.
  • Prepare clinical/progress notes and summaries.
  • Assist patients with activities of daily living while teaching self-care techniques.
Requirements
  • Current state license as an LPN.
  • 1 year experience as an LPN in a home care setting.
  • Graduate of an accredited program of practical nursing.
  • Proof of current CPR certification.
  • Ability to work flexible schedule and travel locally.
Salary Information

We offer a competitive salary of $65,000 per year, paid in installments of $300 after 30 days, $175 after 60 days, and $175 after 90 days. A $650 sign-on bonus is available for new or returning caregivers who bring their own client with at least 30 service hours per week.
