Job Summary

We are seeking a highly skilled Registered Nurse II to join our Med Surg team at Ascension Wisconsin. As a key member of our healthcare team, you will provide direct nursing care to patients in accordance with established policies, procedures, and protocols.

Responsibilities
  • Implement and monitor patient care plans to ensure optimal patient outcomes.
  • Monitor, record, and communicate patient condition as appropriate to ensure seamless care coordination.
  • Serve as a primary coordinator of all disciplines to provide well-coordinated patient care.
  • Note and carry out physician and nursing orders to ensure timely and effective care.
  • Assess and coordinate patient's discharge planning needs with members of the healthcare team to ensure a smooth transition.
Requirements
  • BLS Provider certification from the American Heart Association is required.
  • Licensed Registered Nurse credential from the Wisconsin Board of Nursing or current home state license for multi-state license recognition is required.
Preferred Qualifications
  • Graduate of a board-approved Registered Nurse Program is required.
  • Bachelor of Science in Nursing (BSN) is preferred.
  • RN with approximately 18 months or more experience is preferred.
