Job Summary:

As a Licensed Practical Nurse (LPN) in our Clinic I (Meds) team, you will provide direct and indirect nursing care to patients in an outpatient setting. You will work under the direction of a Registered Nurse and medical orders from a Practitioner to ensure high-quality patient care.

Key Responsibilities:
  • Medical Record Management: Review and prepare medical records for assigned patients, initiate follow-up for medical records as requested, and review patient needs to identify and prepare orders for patient authorization by the Practitioner.
  • Nursing Procedures: Implement basic nursing procedures per plan of care, administer treatments, immunizations, and medications as prescribed by the Practitioner, and perform point-of-care testing as authorized.
  • Documentation and Communication: Document implementation of orders/interventions, record findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards. Observe patients/family responses to the plan of care and report unexpected responses to the RN or Practitioner.
  • Team Collaboration: Collaborate with the RN/Practitioner to revise the care plan when needed, demonstrate and apply competency in basic nursing skills and use of equipment, and maintain current knowledge and skills needed to perform the position.
  • Emergency Response: Perform efficiently in emergency patient care/clinic situations, follow established procedures, and inform the appropriate Practitioner/RN.
  • Leadership and Training: Assign basic nursing tasks to unlicensed personnel as appropriate and according to department/unit standards, provide clear directions, act as a resource person to unlicensed staff, and follow-up on assigned care to ensure it is given per standards, policies, procedures, protocols, and guidelines.
  • Vaccine Management: Manage all patient vaccine data, including updating the Hawaii Immunization Registry System (HIR), Electronic Health Records (EHR), and all patient and/or external records at each patient visit. Read and administer immunizations according to Health plan and VFC guidelines.
  • Compliance and Quality: Ensure compliance with infection control policies and protocols, follow medication administration, storage procedures, Health plan CDC, State, and federal VFC program guidelines, and participate in orientation of new personnel.
Requirements:
  • Education: Hawaii State Board of Nursing approved curriculum for LPN licensure.
  • Experience: Twelve (12) months (2080 hours) LPN experience.
  • Licensure: Licensed Practical Nurse (Hawaii) required at hire.
  • Certifications: Basic Life Support from American Heart Association.
Preferred Qualifications:
  • Acute Care Experience: Acute care nursing experience.
  • Technical Skills: Demonstrated knowledge of and skill in word processing and spreadsheet PC applications.
