Position: Social Work Case Manager

Hours: 8 AM - 5 PM
Compensation: $28/hour

Key Responsibilities:
  • Assess the requirements of clients, evaluate available resources, and propose or facilitate plans for optimal outcomes.
  • Assist in formulating ongoing long-term care strategies and identify necessary providers, specialists, and community resources.
  • Facilitate communication between clients, their families, and the care provider team to ensure accessibility of services.
  • Offer resource support to clients and their families for various needs, including employment, housing, independent living, and justice-related services.
  • Monitor and evaluate care plans and client progress, providing recommendations based on identified needs.
  • Engage with long-term care healthcare providers to ensure comprehensive support for client needs.
  • Document and maintain accurate records of client information and care management activities to comply with regulatory standards.

Qualifications:
  • Minimum of 2 years of experience in case management.
  • Proficient in electronic medical records.
  • Strong computer skills, particularly in MS Office.
  • Bachelor's Degree required.

Additional Requirements:
Must possess reliable transportation for client visits.

Bilingual Requirement: Proficiency in both English and Spanish is essential.
