Clinical Care Manager RN

We are seeking a highly skilled and compassionate Clinical Care Manager RN to join our team at Nursing Solutions. As a Clinical Care Manager RN, you will play a vital role in ensuring the delivery of high-quality patient care and services to our clients.
Job Summary:The Clinical Care Manager RN will be responsible for managing a caseload of clients, developing and implementing treatment programs, and coordinating care with external providers. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Manage a caseload of clients and develop individualized treatment plans
- Coordinate care with external providers, including physicians, hospitals, and other agencies
- Provide training and guidance to direct care staff on orders for disciplines and/or treatments and services
- Ensure referrals result in a timely start of care or are well documented for permitted delays or non-admissions
- Effectively manage a caseload of clients and develop realistic personal goals with patients
- Conduct comprehensive assessment/reassessment visits and document visits
- Provide direct, in-field supervision of newly hired clinicians and assure supervision of all staff is completed per regulations and agency policy
- Provide direct response and guidance to staff for problem resolution in patient care
- Participate in orientation of new staff, in-service education, continuing education, utilization review, and QAPI programs
- Current Registered Nurse (RN) license in good standing and currently licensed
- Graduate of an accredited school of nursing
- Home Health, pediatric, and/or office experience a plus
