Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Rockford Center, a 138-bed mental health facility located in Newark, DE. As a Registered Nurse, you will provide direct nursing care to patients as prescribed by the physician and under the direction of the facility's nursing leadership.

Key Responsibilities
  • Conduct ongoing and individualized nursing care duties as assigned and as dictated by the patient's condition in accordance with physician orders and facility protocols.
  • Administer PRN medications, assess for confirmed med for PRN, document need (rationale) and its effectiveness in the medical record.
  • Report changes in patient physical, mental status or episodes of seclusion or restraint to nurse manager or supervisor during each shift.
  • Ensure family and physician notification is completed and documented if a patient is transported to Emergency Room, Injured, Falls or is Placed in Seclusion or Restraints.
  • Collaborate with other team members in the development, implementation, and evaluation of an individualized plan of care.
  • Perform treatments and therapies as ordered by physician in an appropriate and timely fashion.
  • Document patient care, findings, and psychosocial responses to nursing interventions.
  • Document PRN medications given, reason and effect.
  • Initiate emergency measures according to age-appropriate CPR and client resuscitation protocols.
  • Participate in emergency crisis interventions according to age-appropriate techniques.
  • Maintain confidentiality related to patient, family, client facility and staff in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
