Key Responsibilities- Collaborate with healthcare professionals to develop and implement patient care plans
- Conduct assessments and evaluations to identify patient needs and develop strategies for care
- Provide education and support to patients and their families to promote health and well-being
- Document patient progress and outcomes in a timely and accurate manner
- Participate in interdisciplinary team meetings to discuss patient care and develop plans for improvement
- Master's degree in Social Work from a graduate school accredited by the Council on Social Work Education
- One to two years of post-graduate experience in a healthcare setting
- Current licensure as a Medical Social Worker in good standing
- Ability to spend approximately 20% of work time driving to and from patient locations
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
