Job Summary

We are seeking a highly skilled Nutrition Specialist to join our team at The Chateau at Brooklyn Rehabilitation and Nursing Center. As a key member of our clinical staff, you will be responsible for assessing the nutritional needs of our residents and providing personalized care and support.

Key Responsibilities
  • Assess and Plan: Conduct thorough nutritional assessments of residents to identify their individual needs and develop personalized care plans.
  • Provide Education and Support: Offer nutrition counseling and education to residents and their families to promote healthy eating habits and support overall well-being.
  • Collaborate with Interdisciplinary Team: Work closely with our interdisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and support for our residents.
  • Manage Nutrition Care: Develop and implement nutrition care plans, including enteral nutrition, to meet the unique needs of our residents.
  • Communicate Effectively: Maintain open and effective communication with residents, families, and healthcare team members to ensure seamless care and support.
Requirements
  • Education and Certification: Registered Dietitian or Registered Dietitian Eligible (with DI Verification Statement) required.
  • Experience and Skills: Proven experience in clinical nutrition, with strong knowledge of nutrition care regulations and survey processes.
  • Communication and Interpersonal Skills: Excellent verbal and written communication skills, with the ability to work effectively with diverse individuals and groups.
What We Offer
