Job Summary

Providence is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Medford, Oregon. As a Home Health RN, you will provide quality care to patients in their homes, working alongside an interdisciplinary team to deliver patient-centered care that addresses the physical, emotional, and social needs of our patients.

Key Responsibilities

  • Conduct thorough assessments of patients' health status and develop individualized care plans to address their unique needs.
  • Provide direct patient care, including medication management, wound care, and education on self-care techniques.
  • Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between settings.
  • Maintain accurate and timely documentation of patient information, including progress notes and treatment plans.

Requirements

  • Educational preparation to meet certification, license or registration requirements.
  • Oregon Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • 1 year of medical/surgical experience in an acute care setting required for Home Health RN.
  • 2 years of pediatric experience in an acute care setting for RN caring for pediatric patients.
