About the Role

We are seeking a highly skilled and compassionate Physical Therapist to join our team at Heartland Home Health. As a Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in need of skilled rehabilitation services.

Key Responsibilities
  • Develop and implement individualized physical therapy treatment plans to help patients achieve their maximum potential for independence and mobility.
  • Conduct thorough assessments and evaluations to identify patient needs and goals, and develop effective treatment plans to address these needs.
  • Collaborate with healthcare teams, including physicians, nurses, and other therapists, to ensure comprehensive and coordinated care.
  • Provide education and training to patients and their families on proper techniques and strategies for maintaining independence and managing chronic conditions.
  • Document patient progress and outcomes, and make adjustments to treatment plans as needed to ensure optimal results.
Requirements
  • Master's degree from an accredited Physical Therapy Program.
  • Current licensure as a Physical Therapist in the state of practice.
  • Current CPR certification.
  • One year's experience in hospital or home health preferred.
