ResponsibilitiesAs a Registered Nurse (RN) Case Manager, you'll supervise, evaluate, and provide nursing services to clients in various senior living communities, including assisted living and independent living facilities. You'll work collaboratively with facility wellness staff to ensure strong lines of communication and overall continuity of care for residents and their loved ones.
- Provide care to patients within the beautiful confines of assisted living facilities
- Deliver 1:1 patient care
- Follow each patient from start to discharge
You'll consult with physicians and office staff as needed, providing details about patient care and acting as the point of contact for all disciplines involved in patient care. Accurate OASIS collection, medication profile updates, lab value reporting, and attending weekly case conferences and monthly case manager meetings are key responsibilities.
QualificationsTo be successful in this role, you'll need:
- Current licensure as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience
- Demonstrated knowledge and skill in current nursing practice
- A valid state driver's license and automobile liability insurance
