We are seeking a highly skilled and experienced RN Clinical Manager to join our team at CenterWell Home Health. As a key member of our leadership team, you will be responsible for overseeing the clinical operations of our home health services, ensuring the highest quality of care for our patients.
Key Responsibilities- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location.
- Conduct assessments and reassessments of patients, including updating of care plans and interpreting patient needs.
- Manage the assignment of caregivers and oversee the delivery of care to all patients served by the location.
- Ensure compliance with regulatory agencies and requirements of third-party payers.
- Coordinate communication between team members, attending physicians, and caregivers to ensure the appropriateness of care and outcome planning.
- Supervise all clinical employees assigned to a specific location and carry out supervisory responsibilities in accordance with Company policies and procedures.
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
- Home health experience is required.
- Management and people leadership experience, strongly preferred.
- OASIS experience, required. OASIS certification (HCS-O, COQS, and/or COS-C), preferred.
- Homecare Homebase (HCHB) experience, preferred.
- CMS PDGM billing knowledge and/or experience, preferred.
