We are seeking a compassionate and skilled Hospice Registered Nurse to join our team as an On-Call RN. In this role, you will provide direct care and support to patients and their families, while also coordinating with our interdisciplinary team to ensure the best possible care experience.
Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Initiate and reevaluate patient plans of care as necessary, utilizing health assessment data to determine nursing diagnoses.
- Administer medication and treatment as prescribed by the physician, and counsel patients and families on meeting their nursing and related needs.
- Maintain accurate and detailed clinical notes, and communicate effectively with physicians and other healthcare professionals to ensure seamless care coordination.
- Ensure compliance with all state and federal regulations, and participate in ongoing education and training to stay up-to-date on best practices in hospice care.
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is highly desirable.
- Knowledge of the hospice philosophy of care and a commitment to clinical and documentation excellence.
