The position entails working varying shifts, including evenings and weekends, primarily in Charlotte and DeSoto counties. You will also have opportunities to work in Sarasota and Manatee counties occasionally. As a RN - Triage/Field, you will visit patients in their homes, long-term care facilities, assisted living facilities, hospices, or hospitals, providing them with clinical support and guidance under the direction of a licensed physician.

Your Key Responsibilities will include:
• Performing patient assessments and developing individualized plans of care
• Managing patient discomfort and providing symptom relief
• Educating patients and families on end-of-life care and available resources
• Collaborating with interdisciplinary teams to ensure comprehensive care
• Accurately and timely documenting patient services and outcomes

To be successful in this role, you will require:
• Current, unrestricted Florida RN licensure
• BSN or MSN degree preferred
• Minimum one year recent acute care experience in a hospital or home health environment; previous experience as a Hospice RN preferred
• CHPN certification encouraged within 3 years of employment
